Continuing Care Services are a number of options created to support clients and their family caregivers when they cannot fully take care of themselves. There are three main types of services:
- Home and Community Care - includes home support, home care and respite care. Social support is available in some communities through the Elders Day program and specialist services like chronic care, foot care, palliative care or equipment loans can be accessed through the health center.
- Supported Living - includes Elder’s' independent living with supports, supportive living with day programs, and 24-hour care in a group home.
- Facility Living - includes long-term and extended care support for people who can no longer live in their home. Click here for information on Long-term care facilities in the Beaufort Delta region.
